How to make a Health Insurance claim
Even if you think you may need to make a claim, the first step is to contact us on 0800 346 765 or send an email to firstname.lastname@example.org.
We’ll email you the required health insurance claim form to get the process started.
You’ll need to bring your health insurance claim form when you first see your GP as they’ll need to complete the GP referral section of the claim form.
Before you proceed with your surgery or treatment, you need to seek pre-approval.
You will need to send us:
- A completed health insurance claim form including an estimate of costs
- A copy of your referral or specialist letter
